Sensory deprivation may be instrumental in producing paranoid states and in precipitating or worsening confusion. Poor hearing is also associated with depression. An estimate of visual and auditory acuity is part of the examination of every old person. Wax in the ears is an easily remedied cause of poor hearing.

Speech is also influenced by various motor functions. Slurred speech may be found in the patient who is drowsy or under the influence of drugs or alcohol, and it sometimes also results from specific neurological problems such as a stroke. Patients with multiple sclerosis may produce so-called 'scanning' speech in which words are produced without inflexion and with hesitation between words. Patients with severe Parkinsonism may have difficulty in forming sounds at all (aphonia). Some difficulty in finding words and putting speech together is found in many patients with dementia, particularly those with Alzheimer's disease.

They also occur in severe depressive illness and mania, when they are often consistent with the patient's mood. Hallucinations of touch (tactile), smell (olfactory) and even taste (gustatory) also occur. Hallucinations of being touched (especially those with sexual connotations) occur in schizophrenia, and hallucinations of smell (especially of the patient believing him- or herself to smell 'rotten' occur in severe depression. Experiences similar to hallucinations sometimes occur in bereavement, and vary from 'hearing the footsteps' of the lost person to complex phenomena occurring in more than one modality.
